What we need on file
- General liability: a certificate from your insurance agent with Functional Floors listed as the certificate holder. At your next renewal, also have Functional Floors and Finishing, LLC named as additional insured.
- Workers' comp: your current policy, or if you have no employees, the Michigan Notice of Exclusion (WC-337) with the State's stamp.
